Southlake Health launches new initiative to connect patients with care before hospitalization

New population health program will use data, research and community partnerships to improve access to care.

Southlake Health has launched a new initiative aimed at improving how healthcare is delivered across northern York Region and southern Simcoe County, with a focus on helping patients access services before their conditions become more serious.

The Population Health Management Advance, known as PHM Advance, will bring together clinical expertise, health data, research and community partnerships to identify gaps in care and develop more proactive approaches to supporting patients.

Rather than relying primarily on hospital-based treatment after health problems escalate, the initiative will explore opportunities to identify people who may benefit from earlier intervention and connect them with appropriate services.

The approach is intended to improve health outcomes, strengthen coordination between providers and potentially reduce avoidable hospital admissions.

Using data to improve patient care

Population health management involves examining the healthcare needs of communities and using that information to guide how services are planned and delivered.

Through PHM Advance, Southlake will identify opportunities for improvement, test new approaches, evaluate their effectiveness and expand those that demonstrate positive results.

A dedicated team at the hospital will collaborate with local healthcare providers to develop practical solutions that help patients navigate the health system and receive care in appropriate settings.

The initiative comes as Southlake serves communities experiencing continued population growth and increasing demand for healthcare services.

International expertise supporting local innovation

Southlake has also established a Global Advisory Board featuring healthcare leaders from Canada, the United States, the Netherlands and Singapore.

The board will contribute expertise and perspectives on population health management, helping the hospital evaluate approaches that could improve care delivery.

PHM Advance forms part of Southlake’s broader strategy to develop a Distributed Health Network, which aims to provide care across multiple locations and community settings rather than relying on a single hospital campus.

The model emphasizes bringing services closer to patients, improving connections between healthcare providers and supporting people in maintaining their health for longer.

A shift toward prevention

The initiative reflects a broader effort to move healthcare delivery toward earlier intervention and stronger coordination between hospitals and community-based services.

For patients, that could mean receiving additional support before a manageable health issue becomes an emergency.

For healthcare providers, it offers an opportunity to improve communication, identify unmet needs and direct resources toward patients who could benefit most.

Although the initiative is newly launched and its outcomes have yet to be demonstrated, Southlake intends to use ongoing evaluation to determine which approaches should be expanded.
