Demethoxyrapamycin, also known as RLX-201, is a new skincare ingredient designed to influence one of the cellular pathways involved in skin aging: the mechanistic target of rapamycin, or mTOR, pathway. Demethoxyrapamycin is a derivative of rapamycin, however, demethoxyrapamycin is not rapamycin.Demethoxyrapamycin is a proprietary derivative of rapamycin developed by Rapalogix Health Inc. It is a related but structurally distinct molecule, developed specifically to try to separate out the parts of rapamycin's biology that may be useful for skin longevity from the parts that aren't.

This article will discuss what RLX-201 is and why it is included in the new antiaging serum ReQ.

The science behind RLX-201 is fascinating, but it is also very new and there are not many studies yet looking at it's use topically. (1) However, there is a lot of excitement about this new antiaging serum- so lets unpack the science and see what is known.

RLX-201 is a molecule designed to influence one of the cellular pathways involved in skin aging: the mechanistic target of rapamycin, or mTOR, pathway.

RLX-201 was developed specifically to try to preserve the effects of rapamycin that may benefit skin longevity and help maintain a youthful appearance, while avoiding the undesirable effects associated with broader mTOR inhibition.

The potential skin benefits of RLX-201 were presented by Rahul C. Mehta, PhD, at the Innovation Tank at the 2026 IMCAS World Congress in Paris, France. Mehta is the co-founder and CEO of Rapalogix Health, the company that developed RLX-201 and the parent company behind Re-Q Health. He previously led research and development for SkinMedica and has disclosed multiple patents related to the technology discussed in his RLX-201 presentation.

The results were from a 12-week, double-blind, vehicle-controlled clinical study of 45 women ages 35 to 65 using the Re-Q serum containing RLX-201. Potential benefits reported included:

  • Firmer skin
  • Improved skin elasticity
  • Less visible sagging
  • Improved skin tone
  • Increased skin radiance
  • Smoother skin texture
  • Improvement in the appearance of wrinkles

Because the study evaluated the finished Re-Q serum rather than RLX-201 alone, we cannot attribute every benefit specifically to RLX-201. These clinical results have not yet been published in a peer-reviewed medical journal.

I have just received samples of the serum and have started testing it myself, but I have not used it long enough to give you my personal assessment. I will update this article after I have had more time to evaluate it.

Side-effects of RLX-201

Side Effects and Downsides

The biggest downside of RLX-201 is how new it is. The sole peer-reviewed published study was performed primarily in human dermal fibroblasts, so we do not yet have much published information about potential side effects in people. For now, much of what we know about tolerability comes from early clinical experience and anecdotal reports. So far, I have not heard reports of skin irritation, acne breakouts, or rosacea flares from my patients or colleagues using Re-Q. In fact, one person told me that her redness improved while using it, although this is anecdotal and does not mean that RLX-201 has been shown to treat redness or rosacea. I have DSNW, rosacea-prone skin and react to many skincare products, and so far I have not experienced any redness or irritation from Re-Q myself. We need more clinical experience and published human studies before we can fully understand its side-effect profile.

Contraindications of RLX-201

Re-Q should not be used during pregnancy or breastfeeding. RLX-201 (demethoxyrapamycin) is a derivative of rapamycin, a drug that is contraindicated during pregnancy because of potential fetal risk. Re-Q should also not be applied to areas with an active bacterial, viral, or fungal skin infection, because inhibition of mTOR signaling may interfere with normal immune responses and wound healing. Avoid applying it to open wounds or significantly damaged skin. If you develop signs of infection or a significant skin reaction while using Re-Q, stop using the product and contact your physician.

Skin Care Products with RLX-201

Re-Q Pro-Longevity Face Serum is a new professional skincare product containing RLX-201 (demethoxyrapamycin), a proprietary molecule developed to target the mTORC1 pathway involved in cellular aging. Re-Q is very new and is currently available through only a select network of dermatology and aesthetic practices in the United States. Because the product is so new, clinical experience with it is still limited. I recently received samples and am in the process of testing Re-Q myself, so I have not yet formed a personal opinion about how well it works.

The science behind RLX-201 is intriguing, but I would like to gain more firsthand experience with the product before making definitive recommendations. I'm using it myself.  I will let you know in 6-12 weeks.

Science Behind RLX-201

As fibroblasts age and are repeatedly exposed to stress, some become senescent, meaning they are still alive but no longer function like healthy younger cells. They become less effective at making collagen and maintaining the extracellular matrix that keeps skin firm and resilient.

 One theory of aging is that persistently elevated mTORC1 signaling keeps telling these already stressed cells to grow, make proteins, and remain metabolically active when what they really need is time for cellular maintenance and repair. Turning down mTORC1 is thought to give the cells a metabolic “rest,” allowing them to repair damage and potentially “reset” toward healthier cellular behavior. This idea of a cellular “reset” is one reason mTOR inhibition has become so interesting in longevity research.

RLX-201 is designed to turn down mTORC1 without turning down mTORC2

This distinction is important. mTORC2 supports cell survival, normal metabolism, and cellular structure, functions that we want to preserve. So the goal is not to shut down an aging fibroblast or stop it from working. The goal is to quiet excessive mTORC1 signaling long enough to allow the cell to devote more resources to maintenance and repair, while preserving mTORC2 signaling needed for healthy cell function. In simple terms, researchers are trying to give an overworked aging cell a rest so it has an opportunity to “reset,” rather than turning the cell off altogether.

mTORC1

mTORC1 is like a cellular signal that tells a cell when it has enough nutrients and energy to grow and make things. When mTORC1 is activated, it stimulates protein production, cell growth, and other energy-intensive activities. In a young, healthy fibroblast, this is necessary for normal function, including producing collagen and other components of the skin's extracellular matrix.

The problem may occur when mTORC1 remains chronically activated as cells age. Older fibroblasts accumulate damage and can become stressed, dysfunctional, or senescent, yet mTORC1 may continue signaling them to grow and produce proteins. At the same time, high mTORC1 activity suppresses autophagy, one of the cell's important systems for breaking down and recycling damaged cellular components.

This is why researchers are interested in temporarily turning down mTORC1. It may give an overworked aging cell a chance to stop focusing so heavily on growth and production and instead devote more resources to cleanup, maintenance, and repair. You can think of it as giving the cell a rest and an opportunity to “reset.”

mTORC2

mTORC2 helps regulate cell survival, metabolism, and the cytoskeleton, the internal structural framework that helps a cell maintain its shape and function.

RLX-201 and mTORC1 and 2

The distinction between the roles of the two types of mTORC may be one of the most interesting things about RLX-201. 

The goal of antiaging skin care is to give the fibroblast cell a rest without turning off the systems it needs to recover and function normally.. Traditional rapamycin can affect both mTOR complexes under some conditions. RLX-201 was developed to preferentially inhibit mTORC1 while preserving mTORC2 signaling.
In other words, the goal is not to shut the fibroblast down. Researchers are trying to quiet the mTORC1 signals that may keep an aging cell working when it needs time for maintenance, while leaving mTORC2 available to support the cell's normal survival, structure, and metabolic functions.

Absorption

RLX-201 has a molecular weight of approximately 884 Da, considerably above the traditional 500-Da threshold associated with efficient passive skin penetration. Therefore, an important unanswered question is how much topically applied RLX-201 reaches dermal fibroblasts in human skin. Penetration will depend heavily on the vehicle, concentration, partition coefficient, solubility, and formulation technology used in Re-Q.

Giving aging skin cells a temporary rest may help reset their activity, theoretically allowing other anti-aging ingredients to work more effectively when they are restarted.

Bottom Line

The science behind RLX-201 is credible, interesting, and based on an important area of aging research. Selectively reducing mTORC1 activity while preserving mTORC2 is a compelling approach, and the early laboratory and clinical findings are promising. However, RLX-201 and the Re-Q serum are simply too new for us to know how meaningful the benefits will be with long-term use. There are very few published studies, and much of the clinical data has not yet been published in peer-reviewed medical journals. Anecdotal reports from people using the serum have been encouraging, but these are not a substitute for controlled clinical trials. I am trying the serum myself and will update this article once I have used it long enough to form my own opinion about the results. For now, I consider RLX-201 a promising but still experimental approach to skin longevity and an area worth watching as the fields of wellness, longevity, and anti-aging medicine increasingly focus on ways to improve how our cells age.

Can I use Re-Q with retinoids, vitamin C, growth factors, or other anti-aging products?

Yes. However, RLX-201 may work best without ingredients that stimulate fibroblasts, because the goal of RLX-201 is to decrease mTORC1 signaling and essentially give aging fibroblasts a “rest.” I would stop growth factors and retinoids for a few months while using Re-Q, then restart them afterward. This is based on the science and mechanism of RLX-201 rather than clinical studies, and my recommendations may change as we gain more experience with this new ingredient.

How long should I use Re-Q before deciding if it works?

I would use Re-Q for 2–3 months to give aging fibroblasts a “rest” or “reset,” then restart your usual anti-aging ingredients and evaluate your skin another 8–12 weeks later, about 5–6 months total. The theory is that rested fibroblasts may respond better when anti-aging stimulation is restarted. Some people notice improved radiance sooner, but changes in collagen and firmness take longer to assess. This approach is theoretical and may change as we gain more clinical experience with RLX-201.

Is Re-Q with RLX-201 safe for sensitive skin?

Probably. So far, I have not heard reports of significant irritation, acne breakouts, or rosacea flares with Re-Q. I have DSNW, rosacea-prone skin that reacts to many products, and I have not experienced redness or irritation. However, RLX-201 is very new, so we need more clinical experience.

Should I use Re-Q before a chemical peel, laser, microneedling, or other cosmetic procedure?

Not right before. These procedures stimulate fibroblasts to produce collagen, while RLX-201 inhibits mTORC1 and could theoretically reduce that response. Although this has not been studied, I would use Re-Q for 2–3 months, stop it for at least 2 weeks, and then have the procedure. Theoretically, these “rested” fibroblasts might even respond better to treatment, but this remains a hypothesis.

Can Re-Q cause acne?

So far, I have not heard reports of acne breakouts from Re-Q. However, RLX-201 is too new for us to know its complete side-effect profile. Remember that RLX-201 is not rapamycin, so reports about compounded topical rapamycin should not automatically be applied to Re-Q.
